Frailty renders older individuals more prone to adverse health outcomes. Little has been reported about the transitions between the different frailty states. We attempted to examine the rate of these transitions and their associated factors.

Frailty has important implications for the care needs of older adults and how those needs are met. By recognizing frailty and measuring it objectively, clinicians can better engage patients and their loved ones in difficult discussions about treatment plans and prognosis, and ultimately deliver better palliative care.

Lesbian, gay, bisexual and transgendered (LGBT) people experience numerous issues as a result of discrimination and stigmas. Recent research has highlighted the significant issues impacting aging and end-of-life issues among this population, particularly when accessing health and social services. Specifically, one issue that continues to negatively impact LGBT people is discrimination from healthcare providers, where many LGBT people are asked inappropriate questions, mocked or treated poorly.

Healthcare systems are not well designed to help people maintain or improve their health. They are generally not person-focused or well-coordinated. The objective of this study is to evaluate the effectiveness of the Health Teams Advancing Patient Experience: Strengthening Quality (Health TAPESTRY) approach in older adults. The overarching hypothesis is that using the Health TAPESTRY approach to achieve better integration of the health and social care systems into a person’s life that centers on meeting a person’s health goals and needs will result in optimal aging.
